Vital Care Applications can be picked up at the Wayland EMS Office during business hours or printed. Completed Applications may be returned in person during business hours or by mail. Payment is excepted in cash, check, or card form. Vital Care memberships cost 45 dollars a year or you can pay now for 3 years and get a 4th year free (135 dollars).
